Following the Government's announcement that the wearing of face masks in shops will be compulsory from Friday 24th July, we are receiving requests for letters of exemption.

The current government guidelines do not currently require an individual to provide evidence of exemption and as such, we are unable to provide letters regarding this.

If you are unable to wear a face covering, there are a number of websites from which you can print an exemption notice that politely advises others as to why you are not wearing a face covering.

Online 'Pre-Registration' With The Practice

If you wish to pre-register, please complete both the on-line Pre-registration Form and Medical Questionnaire. When you have completed all of the details, click on the "Send" button to email your form to us. When you visit the surgery for the first time you will be asked to sign the forms to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
